This form is for Offcial Emergency Stations to file their monthly reports. Reported information will be forwarded to your coordinator.

You should already have a close relationship with your coordinator, so if you prefer to report directly to your EC or DEC (which ever recommended you for appointment).

Created specifically for KY ARES use, this form may be tailored in the future to better capture data needed by local, district and/or section level coordinators.

If you do not select your county, your report will not be filed. So, please be sure to do so!
